README

1. Use index.php as a starting point. 
2. You must include the headers on index.php on any other forms you may want to create.
3. Do not make the database table ahead of time, although if it is already created, please ensure the field names on the table match exactly to the input names on your form.
4. At the bottom of formdisplay.class.php it is possible to have the form emailed to you and/or the user.
